About HSI Housing and Services, Inc. (HSI) is a nonprofit supportive housing organization that provides permanent housing and life-changing services to more than 700 formerly homeless individuals and families across New York City. We operate four congregate residences and a 100-unit Scatter Site program, and we are committed to providing safe, dignified, and well-maintained housing for our tenants. Kenmore Hall is a 325 unit supportive housing facility located in the Flat Iron District. The project serves a diverse group, including formerly homeless men and women, older adults, and veterans, many of whom are living with chronic health issues, substance use, and mental health disorders. Position Summary We are seeking a compassionate and dedicated Licensed Social Worker to join our team. This role provides an opportunity to make a meaningful impact on the lives of individuals living with HIV who have experienced homelessness and face co-occurring challenges, including chronic health conditions, mental health disorders, and substance use. As part of our supportive housing program, you will empower clients to achieve increased stability, wellness, and independence. The ideal candidate will bring a trauma-informed approach, a strong commitment to advocacy, and a passion for fostering resilience in marginalized communities. This position involves close collaboration with an interdisciplinary team, government agencies, community organizations, and healthcare providers to deliver comprehensive, client-centered care. Key Responsibilities Provide individual counseling and psychosocial support to residents living with HIV, mental health challenges, and substance use disorders. Conduct comprehensive biopsychosocial assessments to identify client strengths, needs, and barriers, and develop tailored service plans. Deliver crisis intervention to de-escalate situations and connect clients to immediate resources. Assess activities of daily living (ADLs) during routine home visits and coordinate referrals to home care, medical services, and other supports. Supervise and mentor social work interns or junior staff members, fostering their professional growth. Collaborate with external partners, including community agencies and healthcare providers, to ensure coordinated and effective service delivery. Maintain accurate, timely, and confidential documentation in compliance with organizational and regulatory standards. Advocate for clients' rights to access necessary services and support their engagement in care. Facilitate peer support groups and therapeutic workshops as directed. Conduct quality assurance reviews to ensure program compliance and service excellence. Assist with administrative responsibilities, including data entry, reporting, and special projects as assigned by your supervisor. ________________________________________ Qualifications Master's degree in Social Work (MSW) or a related field; LMSW or LCSW licensure is required. Proven experience in crisis management and intervention strategies. Expertise in addressing the needs of individuals living with HIV, mental health disorders, and substance use challenges. Familiarity with harm reduction, trauma-informed care, and strengths-based approaches. Strong supervisory skills with the ability to guide and develop junior staff. Excellent verbal and written communication skills. Demonstrated ability to work both independently and collaboratively within a multidisciplinary team. Commitment to continuous learning and adherence to professional ethical standards, including the NASW Code of Ethics—specifically client care, boundaries, confidentiality, and termination of services.
