Licensed Practical Nurse (LPN) Better Living at Liberty Court – Dixon, Illinois “To love generously, serve passionately, and elevate every moment.” Position Summary At Better Living at Liberty Court, our LPNs are more than clinical caregivers—they are the heartbeat of our mission. You partner closely with residents, families, and your team to ensure care is compassionate, thorough, and aligned with Illinois Department of Public Health (IDPH) Assisted Living regulations. Essential Duties & ResponsibilitiesResident Care & Clinical Responsibilities Provide high-quality nursing care in accordance with IDPH Assisted Living & Shared Housing Establishment regulations. Administer medications and treatments per physician orders, documenting accurately and timely. Monitor residents’ health status, observe changes, and promptly notify the Health & Wellness Director (HWD), Executive Director (ED), families, and providers as required. Support residents experiencing chronic conditions, acute changes, or behavioral needs with patience, empathy, and professionalism. Complete clinical documentation, progress notes, assessments, and communication logs following Better Living and IDPH standards. Assist with service plan reviews and updates. Ensure proper infection control practices are followed at all times, including universal precautions and IDPH-required protocols. Medication Management Administer medications safely according to state regulations and Better Living policy. Document all medication administration in real time. Report medication errors immediately to the HWD per policy and support root-cause review. Maintain accurate narcotic counts, medication cart organization, and secure storage. Collaboration, Communication & Leadership Serve as a culture carrier—modeling kindness, professionalism, respect, and Better Living’s mission. Communicate warmly and consistently with residents and their families, ensuring they feel informed and supported. Work as a collaborative partner with caregivers, coaching and guiding them when needed. Respond to emergencies appropriately, including falls, changes in condition, and incidents requiring outside medical support. Participate in daily stand-ups, care meetings, and interdisciplinary discussions. Support onboarding and orientation of new care team members. Regulatory & Safety Responsibilities Maintain awareness of IDPH assisted living regulations and follow all required procedures. Support Life Safety, infection control, and emergency preparedness protocols. Ensure accurate incident reporting and follow-up per policy. Protect resident rights, dignity, autonomy, and privacy at all times. Qualifications Current Illinois LPN license in good standing. Previous experience in senior living, memory care, assisted living, or skilled nursing preferred but not required. Strong relationship-building skills with residents, families, and teams. Ability to respond calmly and effectively in emergency or stressful situations. Compassionate heart, strong clinical judgment, and commitment to excellence. Schedule & Reporting Reports to: Health & Wellness Director Works collaboratively with: Executive Director, Caregivers, Activities, Dining, and the entire Better Living community.
