Home Health Registered Nurse (RN)We are seeking an experienced Home Health Registered Nurse (RN) to provide comprehensive skilled nursing care to patients across a large multi-county service area. This role is ideal for a confident home health RN who is experienced in OASIS, enjoys clinical variety, and is comfortable managing all visit types while working independently in the field.Schedule & Work DetailsSchedule: Weekdays, 8:00 AM 4:30 PMOn-Call / Weekends: Required participation in weekend and on-call rotationsMileage Reimbursement: Yes reimbursed between visits, from first to last patient of the dayDress Code: Scrubs (any color)Key ResponsibilitiesPerform comprehensive home health nursing visits across the entire service areaComplete all visit types, including:Start of Care (SOC)Resumption of Care (ROC)OASIS recertificationsEvaluations and follow-up visitsRoutine and PRN visitsCheck-in callsTransfers and dischargesConduct detailed patient assessments and monitor changes in conditionDevelop, implement, and revise individualized plans of care based on patient needsProvide skilled nursing interventions and education aligned with physician ordersCollaborate with physicians and interdisciplinary team members to ensure continuity of careAccurately complete and submit clinical documentation, including OASIS, in compliance with regulatory guidelinesParticipate in on-call and weekend coverage as scheduledTravel daily between patient homes throughout assigned countiesRequired QualificationsActive Registered Nurse (RN) licenseHome health nursing experience requiredOASIS experience requiredStrong clinical assessment, documentation, and organizational skillsAbility to manage diverse visit types independently in a field-based rolePreferred Skills & Attributes (Nice to Have)COS-C (Certificate of Completion OASIS-C)High attention to detail in documentation and care coordinationStrong time-management skills in a productivity-driven environmentEffective communication with patients, caregivers, and clinical teamsFlexible, reliable, and patient-focused approach to care
