Job Description Job Opportunity: Physical Therapist - Home HealthOur organization is on the lookout for a dedicated and empathetic Physical Therapist to join our Home Health team. In this position, you will deliver personalized rehabilitation services to patients in their own residences, helping them regain mobility, enhance functionality, and foster independence.Key ResponsibilitiesConduct thorough initial and ongoing assessments of patients.Establish functional and measurable goals tailored to each individual.Implement therapeutic exercises and manual therapy techniques.Educate patients and their families on safe mobility practices and the use of assistive devices.Regularly evaluate patient progress and adjust care plans accordingly.Document all visits, interventions, and patient responses accurately and promptly in the electronic medical record.Ensure compliance with regulatory standards and payer guidelines.Prioritize patient safety and adhere to infection control and home safety protocols.Provide education to patients and caregivers to encourage self-management and prevent falls.Manage a caseload within a designated geographic area, planning efficient routes and communicating any schedule changes effectively.Requirements and QualificationsLicensed Physical Therapist with relevant experience.Ability to work independently in a home health setting.Strong interpersonal skills to build trust with diverse patients and families.Self-motivated and organized, with excellent time management skills.Working ConditionsThis role involves traveling to patients' homes within a specified area.
