School Based Clinician

his position requires strong organizational skills and the ability to manage multiple priorities, attention to detail and excellent communication ls. Implement phil phy and objectives of the Agency. Assess, diagnose and treat clients according to current evidence based and/or best practice guidelines. Independently conduct comprehensive assessments makes decisions, and plan client care. Perform tasks independently, consulting with supervisory team and peers as needed. Work with a diverse group of clientele. Document sions and summary in ctronic medical record database within agency-specified meframes. Participate in general agency and model-specific training (including Risking C nection) Report suspected child abuse and neglect in collaboration ith supervisor within the 12-hour time frame (verbally) and 48-hour time frame (written) Attend a participate in weekly supervision Service Intensity: 1-2x week individual, group family therapy Engage caregivers & parents in treatment according to case formulation and treatment plan Weekly gro sessions if group therapy is part of client treatment plan Hold parent-guidance sessions as clinically indicat Meet productivity expectations as defined by the program leadership team Complete thoroug assessment and be able to diagnose using the DSM-5 Create treatment plans and documentation in collaboration th families Complete worker and youth Ohio Scales at intake, eac pdate, and disc ge (or at least 90 days prior to discharge) Will be trained in at least one or more Evid ced Based Practice (EBPs) and will use EBPs when clinically ap opriate. Will fully complete all EBP related assessments and reoccurring measures. Collaborate with school, clinical team, age y psychiatrists, primary care provider, and external agencies (i.e. DCF, past providers) Participate in weekly case review and individual supervi on with supervisor Will achieve and maintain “Green” QA Level III statu in regards to all chart documentation throughout employment Will maintain 90% of billable expectation over the fiscal year 70% of clients receiving tal health s ics through the SHC for at least 3 months will demonstrate improved psychosocial f ctoning (per DPH/Unite Way contract) 100% of referrals to the SBHC clinician will be reviewed within 30 days of initial re rrl Will have properly doumented client signature forms on all summary documentation 100% of the time Benefits clude: 3 weeks pai vacation time 12 sick days 12 paid holidays 5 personal da Wnter limited holiday hutdown period Medical & Dental Insurance 403(b) 403(b) matching Bi-lingual Spanish/English preferred w significant differential offered.
