Job Title: RN, Care Manager (Home Health)Pay rate:Weekly Gross: $2800.00(Weekly Stipend non-taxable: $2000.00Taxable Weekly:$800)Blended Hourly: $70.00Shift:Days 8:00 AM to 4:30 PM40 hours/WeekProvides excellent patient care by assisting in collaboration, development, implementation, revision and reporting of the case management program.Acts as a liaison between the patient, family, nurse, physicians, multidisciplinary team and patient's healthcare benefactor to optimize outcomes.Serves as a consultant to the healthcare team on specific patient items.Day Shift,40 hours per week, coverage area is Lake County, CA; mileage (government rate) is in addition as travel is needed to see patientsTHIS IS FOR HOME HEALTH NOT HOSPITAL, Home Health experience is A MUST/REQUIRED.MUST have HCHB software experience is requiredEducation and Work Experience: Bachelor's Degree in Nursing (BSN): PreferredExperience in a care management role: PreferredLicenses/Certifications: Registered Nurse (RN) licensure in the state of practice: RequiredCase management certification: PreferredBasic Life Support (BLS OR HS-BLS OR RQI BLS) certification: RequiredTo Apply:
