This role focuses on symptom management, coordination of interdisciplinary services, patient and family support, and end of life care aligned with hospice philosophy.Schedule & Work DetailsCoverage Areas: Clallam and Jefferson Counties, Western WashingtonSchedule: Monday through Friday, 8:00 AM to 4:30 PMWeekly Hour Guarantee: 32 hoursOn Call/Weekend Requirement: Occasional coverage when night or weekend staff call offMileage Reimbursement: Reimbursed between patient visits only; commute to and from work is not reimbursedPay Rate: $51 to $53 per hourKey ResponsibilitiesManage a caseload of hospice patients as an RN Case ManagerComplete comprehensive hospice assessments and ongoing reassessmentsDevelop, implement, and update individualized hospice plans of careProvide skilled nursing care focused on pain control, symptom management, and patient comfortCoordinate care with physicians, social workers, chaplains, aides, and the interdisciplinary hospice teamEducate patients and families on disease progression, medications, and end of life expectationsSupport patients and families emotionally throughout the hospice journeyEnsure timely, accurate documentation in accordance with hospice regulations and agency standardsMaintain compliance with state, federal, and hospice specific guidelinesRequired QualificationsActive Registered Nurse license in the state of WashingtonPrior experience as a Hospice RN Case ManagerStrong clinical assessment and case management skillsAbility to work independently in the fieldValid driver’s license and reliable transportationPreferred QualificationsExperience with Homecare Homebase (HCHB)Strong communication, organization, and time management skillsCompassionate approach to end of life carePride-Health offers eligible employee’s comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance and hospital indemnity), 401(k)-retirement savings, life & disability insurance, an employee assistance program, legal support, auto, home insurance, pet insurance and employee discounts with preferred vendors.Equal Employment OpportunityPride-Health is an equal opportunity employer.
