Together, we can create a better future for our residents and their families.Position OverviewWe are seeking an experienced and dedicated Post-Acute Care Attending Physician to provide compassionate, proactive medical care across multiple post-acute and long-term care facilities in Maine. This role is available in two geographic areas:Central / Mid-Coast Maine service areaPortland / Southern Maine service areaYou will serve as the attending physician across approximately 4–5 facilities, working closely with interdisciplinary teams and supported by a Medical Director who oversees 8–10 buildings. This model ensures collaborative leadership, local support, and shared accountability for patient experience and outcomes. This role offers a structured, office-like weekly schedule — no overnight shifts, no on-call duties, and no weekend or holiday coverage. Physicians can focus on thoughtful, proactive care while maintaining exceptional work‐life balance.Key ResponsibilitiesServe as primary attending physician across 4–5 skilled nursing and long‐term care facilities within your assigned region.Complete post‐acute admissions, including medication reconciliation, capacity and code status review, and transitional care documentation.Perform weekly SNF follow‐up visits to ensure quality care for our highest acuity patients.Conduct acute change‐in‐condition assessments when needed.Complete discharge visits and summaries when APP support is unavailable.Partner closely with nursing and leadership teams to maintain continuity and communication.Actively support programs in dementia care, wound care, chronic disease pathways, preventive medicine, and infection mitigation.Participate in interdisciplinary care meetings, medical provider huddles, and quality initiatives.Your presence will drive improved care coordination, reduction of unnecessary acute transfers, family satisfaction, and measurable quality outcomes.QualificationsWe seek a dedicated MD/DO with an active medical license in Maine (or eligibility to obtain one) to join our team.
