Case Coordinator - Crisis Stabilization Unit (CSU)
Brief DescriptionSummary: To assist clients on the CSU obtain aftercare services upon their completion of treatment, develop and coordinate treatment plans, and advocate for clients in various community settings. Provides crisis assistance as needed. Assists consumer in maintaining eligibility for applicable programs and conducting required evaluations within timelines specific to applicable program regulations Coordinates and chairs the Interdisciplinary Team to ensure Individual Treatment Plans are developed for each consumer and reviewed as required. Conducts intake assessments for consumers upon arrival on the CSU Regularly provides client transports within catchment areaAdvocates for and assists the Consumer in obtaining needed services through referrals to other programs that meet the Consumers’ needs. Conducts process groups, educational, chemical dependency, and other groups as assigned daily. Demonstrates knowledge of chemical dependency and addictions and utilizes knowledge and skills to access and meet the client’s treatment needs and requirements. Complies with federal, state and company policies, procedures, and regulations. Maintains confidentiality of all consumer information and comply with HIPAA regulations Demonstrates knowledge of and supports Westbrook Health Services mission, vision, values, policies and procedures, operating instructions, confidentiality standards and code of ethical behavior.
