SummaryThe Manager, Social Work is responsible for planning, organizing, and directing the personnel providing social work intervention to patients and their families based on the assessment of individual social, psychological, and economic needs. Directs the identification, assessment, treatment, and referral of patients and their families in need of social work intervention. Hold 24-hour accountability for these department operations.Provides development and oversight of social work and transitional care. Supports actions and processes to ensure the coordination and continuity of health care as patients transfer between different venues of care. Serves in a leadership role in developing interfaces with clinical staff across the continuum and directly supervises Social Workers, in the acute hospital (ACH, SBH, STH) and ensures their competency, productivity and efficiency. Works directly with the System Director Acute Care Management and the Manager Transitional Care Coordination to collaboratively achieve quality care outcomes for patients as they transition from one setting to another.Minimum QualificationsFormal Education Required:Master of Social Work required, along with required Ohio licensure.
