On a day-to-day basis, the Case Manager will conduct assessments, gather and review documentation, and maintain accurate case records. This role includes coordinating services, scheduling and leading virtual meetings, and communicating regularly with clients, internal teams, and external partners to ensure progress and timely outcomes. The Case Manager will monitor milestones, identify risks or barriers, and propose solutions that align with company standards and client goals. The role also involves preparing reports, maintaining compliance with applicable policies and procedures, and contributing to continuous improvement of case management workflows.Qualifications Strong case management and coordination capabilities, including organizing information, tracking deadlines, and managing multiple cases concurrently.Excellent communication skills, with the ability to explain complex information clearly, listen actively, and maintain professional written and verbal correspondence.Demonstrated ability to work independently in a remote environment, including time management, self-motivation, and comfort with virtual collaboration tools.Attention to detail and sound judgment in reviewing documentation, maintaining accurate records, and supporting decision-making.Customer or client service mindset, with experience building trust, addressing concerns, and advocating for client needs.Proficiency with standard office software and case management or CRM systems; ability to learn new digital tools quickly.Relevant experience in case management, social services, project coordination, or a related field; experience in a remote setting is beneficial.Associate or bachelor’s degree in a related discipline (such as social work, psychology, business, or human services), or equivalent practical experience.
