Physical Therapist - In-House SNF (Per Diem)Job Position:Physical therapist for anIn-houseSkilled Nursing FacilityJob Type: Per DiemJob Purpose:To assist in planning, organizing, developing, and directingPhysical therapistservices in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by DOR, Administrator, or Regional support team to assure the highest degree of quality resident care is always maintained.Job Functions:Provide Physical therapistservices to residents in accordance with established standards of practice, Company procedures, Therapy Department procedures and productivity standardsAdhere to Company procedures regarding documentation and billing of Physical therapistservicesDemonstrate sound judgment in the evaluation, planning, implementation, and follow-up of resident therapy programsConduct screening of residents at regular intervals to determine the need for intervention/treatmentConduct timely screening of residents referred to physical therapy to determine the need for intervention/ treatmentEvaluate residents to obtain data necessary for treatment planning and implementationConduct specialized evaluations as indicatedDevelop treatment plans by establishing short and long-term goals and methods to achieve identified goalsInterpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/ responsible parties, and interdisciplinary team membersTo complete compliant and medically necessary documentation from CMS expectations in Net Health EMR systemsEducation & Requirements:Minimum of Bachelor of Science Degree in Physical Therapyfrom an accredited programMust have an active Physical Therapy license in state(s) of practiceOngoing continuing education and professional development to maintain current licensure and certification -Assume responsibility for ongoing continuing education and professional development to maintain current licensure and certificationBenefits:Competitive pay ratesMedical, Dental, VisionPowered by JazzHRLtGm8vnv95
