Join us and be part of something bigger - helping to simplify health care one person, one family and one community at a time.mpany: Oak Street HealthTitle: Supervisor, Clinical Documentation (Full-time)Location: Remote / CentralRole DescriptionThe goal of this position is to support all care teams across the organization in comprehensive and targeted chart reviews -- to ultimately facilitate efficient and effective medical care for our patients. In addition, this role will partner with regional and central leaders, particularly Population Health Directors, Senior Medical Directors, Senior CISs, and CDS program leaders, to support chart review workflow optimization. The Supervisor acts as an in-role expert on all CDS related matters including: training, ongoing audits and assistance with implementation of any new CDS workflows and queries. In addition, the Supervisor will engage with the Manager, Clinical Documentation to facilitate successful onboarding of new CDSs. The Supervisor will report to the Manager, Clinical Documentation.Responsibilities:Lead a team of up to 8 CDSsOngoing audits and feedback for CDSs in podRoutine 1:1sPoint person for suspecting questions from CDSs within their podConduct bi-weekly pod meetingsNew CDS onboardingQuarterly Slide Training for CDS Onboarding will be shared between Supervisors with rotation based on new hire start dateDocumentation Reviews and SuspectingChart reviews for high priority / new providers / first WRVs / targeted listsAttendance at targeted Clinical Documentation Reviews as neededPartnership with Population Health Directors on general chart review support and strategies for regionMedical RecordsAccess and review HIE/EMRs to identify potential suspects based on test results, imaging reports, etc.Identify need for medical records requests from patient information, including payer dataBe an expert in tracking down and leveraging external resources, such as hospital records, Blue Button, and payer data, to support retrospective and prospective accurate documentationIdentify regional trends and training opportunities to support clinical documentation accuracy improvementIdentify and analyze opportunities for improving patient quality outcomes and cost of careAnalyze chronic condition prevalence based on patient demographics and identify opportunities for interventionsStructure initiatives to drive improvement in quality and risk adjustment documentation resultsMonitor clinical documentation effortsDevelop predictive analytics insightsOther duties, as assignedWhat we're looking for:At least 6 months experience as a CDSEase and expertise working with multiple technical systemsAbility to collaborate across multiple teams and engage in multiple perspectives to work toward a common goalProven experience and ability to build relationships with ProvidersAbility to succeed in a fast-paced, ever-changing environmentSelf-starter with high degree of drive, initiative, and follow throughAbility to develop, plan, and implement short-and long-range goalsExceptional interpersonal, written and verbal communication skillsCommitment to company visionExpert in accurate and specific documentationUS work authorizationSomeone who embodies being "Oaky".What does being "Oaky" look like?Radiating positive energyAssuming good intentionsCreating an unmatched patient experienceDriving clinical excellenceTaking ownership and delivering resultsBeing relentlessly determinedWhy Oak Street Health?Oak Street Health is on a mission to "Rebuild healthcare as it should be'', providing personalized primary care for older adults on Medicare, with the goal of keeping patients healthy and living life to the fullest.
