Nurses who thrive here value close patient relationships, clinical excellence, and meaningful work.What You Will DoAs a vital member of our home health team, you will:* Provide skilled nursing care tailored to each client's unique needs* Monitor health conditions and document changes in clinical status* Administer medications and perform wound care* Deliver G-tube, trach, or vent support when required* Educate and guide clients and families to support ongoing health at home* Communicate effectively with the care team to ensure continuity of careWho We Are Looking For* Licensed as an LPN and in good standing with the Georgia Board of Nursing* Experienced in a clinical setting, with at least one year preferred* Certified in CPR* Skilled in EMR documentation, with strong organizational ability* Confident working independently in a home environment* Compassionate, client-focused, and a strong communicator* Experienced with G-tube, trach, or vent care* Licensed to drive with reliable transportationWhy Work With AccordCareWe prioritize both patient well-being and nurse satisfaction.
