Employment Site Check Coordinator
JOB SUMMARY:
To provide direct care and personal adjustment services for individuals participating in the Supported Employment program. Direct care includes job coaching, skills training, assessment, counseling, intervention, and advocacy. Case management, documentation, and employer contacts are also required along with other services needed to maintain the employment of a Supported Employment participant.
ESSENTIAL JOB RESPONSIBILITIES
Performs jobcoaching duties for participants assigned to their caseload. Serves as a positive role model to participant and employer, modeling appropriate work skills, behavior, and social interaction.
Utilizes a variety of jobcoaching techniques to meet the participant and employer needs. To include identification and analysis of work to be performed, aiding the participant in mastering job duties, ensuring that work performed by the participant meets employer expectations, documentation of progress, noting stabilization, and recommending transition to followalong services.
Provides followalong interventions to participants assigned to caseload; a minimum of two contacts per month supported by documentation (or as outlined in CSA). Interventions may include counseling, advocacy, job coaching, skill training, assessment, and referral.
Uses a team approach to share information on participant progress or other relevant information. To include Supported Employment Individual, the Supported Employment Director, family, advocate, and other support members. Participate in developing and reviewing biannual Supported Employment Work Plans to participants assigned to your caseload.
Participate in training to improve skills in the use of structured training techniques for teaching job performance skills, job related skills, and work behaviors. Provide supervision of program participants at work sites not to exceed 8 program participants per work site.
Responsible for documentation of services provided, case notes, notable occurrence, processing of time sheets, and attendance records to ensure compliance with federal, state, and agency regulations.
ADDITIONAL RESPONSIBILITIES
Must possess sufficient stamina to work under all types of weather conditions.
Respond to all requests for specific work services in a timely manner and be available for oncall services according to the schedule. Position requires the ability to be available to work nights and weekends as necessary to meet the onsite checks for all clients supported.
Must maintain eligibility to drive agency vehicles. Loss of privilege means loss of position. Must transport program participants to and from work as required in personal vehicle
Ability to meet frequent deadlines and handle stressful, hectic situations without loss of efficiency and composure
EDUCATION / EXPERIENCE
Associates degree in Human Services or related field with 3 years work experience, or a combination of education and work experience that include a high school diploma and 5 years experience which demonstrates ability and aptitude to provide direct care to participants in Supported Employment.
Basic fluency in English for written and oral
Computer/email skills required
ADDITIONAL REQUIREMENTS
Site Check Coordinator will be required to participate in a general orientation to MidState Arc, Inc. The Policies and Procedures of the Arc, as they relate to the clients of Supported Employment as well as to the employees, will also be presented.
A valid CT driver’s license, 3 years of an acceptable driving record and registered vehicle. We supply mileage reimbursement.
A background check will be performed and Motor Vehicle Record annually.
Participates in any Agency required trainings.
Smart phone and/or device with data plan for daily documentation off site while at community job sites
