(Client minimum: Associates Degree accepted.)Active Kentucky Physical Therapist license required to start the assignment.Home health experience required. Must be proficient with HCHB electronic documentation.Experience performing OASIS assessments, admissions, recertifications, 30-day assessments, and discharges.Experience supervising PTAs.BLS certification from AHA or ARC may be required.Preferred: prior experience working in Logan and Todd county coverage areas.\n\nAssignment Details\nFacility Type: Home HealthShift Name: DaysShift Schedule: Monday - Friday, approximate daytime schedule (am to 4:30 pm) as indicated by the client.Weekly Guarantee: 32 hours per week.Productivity Expectation: 30 points.Coverage Areas: Logan County and Todd County, KY.Mileage reimbursement between patient visits from first to last patient: Yes.On-call/Weekend Requirement: No.Dress Code / Scrub Color: Royal blue.Floating Requirement: Candidate may be required to cover assigned geographic areas as needed.\n\nLocation Highlights\nRussellville, KY provides access to regional parks, outdoor recreation, and a small-city pace of life.
