This role is ideal for a seasoned Case Manager with a background in developmental services who is passionate about delivering high-quality, person-centered care for individuals with developmental disabilities.What We OfferCompetitive salary and comprehensive benefitsGenerous PTO403(b) retirement planTuition reimbursementCareer growth opportunities — one-third of our positions are filled through internal promotionsMission-driven work with direct impact on individuals with developmental disabilitiesCollaborative, supportive team culture dedicated to compassionate, evidence-based careWhat You'll DoThe Case Manager is responsible for coordinating and monitoring service delivery for individuals served in Support Services by completing case management functions, facilitating person-centered planning, and ensuring required documentation and communication with internal and external stakeholdersResponsibilities IncludeInitiating, supporting, and promoting quality of life for clients through empowerment, safety awareness, respect, and dignity.Coordinating and participating in Health Care Management meetings, accompanying clients to medical appointments, arranging necessary follow-up appointments, and collaborating with Staff Supervisors and the nursing staff.Coordinating medications-related documentation in accordance with agency policy and MAP regulations.Providing crisis intervention and emergency services as needed (including participating in the on-call process as needed).Recognizing and reporting concerns related to abuse/neglect, health, and safety.Completing all documentation required in the client's electronic health record.Completing case management forms and other required documents in a time-sensitive manner.Other tasks as required by managementWhat You'll NeedAssociate's Degree (or equivalent) OR High School Diploma / GED AND 3 years of relevant experience (Bachelor's Degree preferred)2+ years of experience in human servicesExperience with case management, documentation, and coordinating services in a human service settingStrong verbal and written communication skillsWe Hire for HEART!Since 1938, BHN has delivered high-quality, culturally responsive behavioral health care across Western Massachusetts.
