Connected Care Coach

A company is looking for a Connected Care Coach to drive behavior changes and positively impact clinical outcomes for members. Key Responsibilities Conduct member outreach to promote behavior change and improve clinical outcomes Utilize a coaching approach to empower members in self-managing chronic conditions Maintain accurate case documentation and complete essential administrative duties Required Qualifications Bachelor's Degree in a relevant discipline or MA/EMT/Paramedic certification 2+ years of experience in a healthcare delivery or related environment Ability to work 1:45pm-10:15pm CST Flexible and able to work in a fast-paced environment Bilingual in Spanish and English is preferred
