LTSS Service Care Manager (RN) – Hybrid Field Case ManagementLocation: New Castle County, DelawareSchedule: Monday–Friday | 8:00 AM – 5:00 PM ESTAssignment Length: 6-Month Contract with Potential for Extension or ConversionWork Setting: Hybrid – Field & RemoteTravel: Local member visits requiredWe are seeking an experienced and compassionate LTSS Service Care Manager (RN) to support long-term services and supports (LTSS) members through comprehensive care coordination, level of care redeterminations, and transition of care activities.This role offers the opportunity to make a direct impact by helping vulnerable populations maintain access to critical healthcare services and community resources while supporting a growing care management team.Position OverviewThe LTSS Service Care Manager assists in developing, coordinating, and monitoring holistic care management activities to improve quality outcomes for long-term care members. This position combines field-based member visits with remote care coordination and documentation responsibilities.The ideal candidate is organized, clinically knowledgeable, independent, and experienced in working with older adults and individuals with disabilities.Key ResponsibilitiesConduct member assessments and level of care redeterminationsDevelop and support individualized care plans and service plansCoordinate care between members, caregivers, providers, specialists, and community resourcesPerform home and community visits to assess member needs and functioningSupport transition of care activities and ongoing case managementEvaluate available resources and recommend services for optimal outcomesEducate members and caregivers regarding healthcare benefits, procedures, and available servicesMonitor member progress, care plans, and outcomes and recommend updates as neededConduct medication reconciliation and review clinical informationMaintain accurate and timely documentation in compliance with regulatory requirementsCollaborate with interdisciplinary healthcare teams and LTSS partnersAssist leadership in identifying opportunities to improve care delivery and member outcomesDaily ResponsibilitiesConduct member home visits and assessmentsComplete level of care redeterminationsSupport LTSS case managers with ongoing care coordination activitiesDocument care management activities and member updatesManage assigned caseload and maintain productivity expectationsPerformance ExpectationsComplete 4–5 member assessments per weekComplete 4–5 level of care redeterminations dailyMaintain timely and accurate documentationMeet quality and compliance standardsRequired QualificationsBachelor's degree and 2–4 years of related experience OR equivalent experienceActive Delaware RN License or Compact RN LicenseValid driver's licenseExperience with:Level of Care RedeterminationsTransition of CareCase ManagementStrong computer literacy and documentation skillsExperience evaluating older adults and/or disabled populationsExperience with medication reconciliationAbility to work independently in a remote and field-based environmentDedicated home workspace for remote work responsibilitiesPreferred QualificationsPrevious LTSS or Service Care Manager experienceExperience working with Medicaid or managed care populationsFamiliarity with community-based resources and long-term care servicesTop Required SkillsComputer literacy and documentation proficiencyExperience evaluating older adults and disabled individualsMedication reconciliation experienceTeam & Work EnvironmentThis role supports a collaborative LTSS team focused on helping members maintain compliance with level of care requirements while ensuring continuity of care.
