Job Title: Registered Nurse (RN) – Skilled Nurse Intakes (Home Health) - $55 per visit Location: Worcester, MA 01604 (Hybrid – Field, Office and Remote Charting) Job Type: Part-time | Contract Compensation: $55 per visit (based on service type and experience) Expected Hours: Flexible. Up to 20 visits per week. 24 to 32 hours per week if desired. We are actively hiring experienced and self-motivated Registered Nurses (RNs) to conduct skilled nursing intake visits and support new patients entering our home-based care programs. Job Description As a Skilled Intake Nurse, you will provide direct patient care and conduct comprehensive nursing assessments for new admissions. You’ll play a key role in educating patients and families, initiating individualized care plans, and ensuring seamless coordination with interdisciplinary team members. Responsibilities Conduct initial skilled nursing assessments in the patient’s home Monitor patients’ health conditions and identify care needs Interpret clinical findings and collaborate with physicians as needed Complete intake documentation accurately and in a timely manner Administer medications and treatments per physician orders Educate patients and families on medications, safety, and care plans Ensure compliance with safety and infection control protocols Communicate effectively with interdisciplinary teams Promote patient well-being through compassionate, high-quality care Requirements Active RN license in Massachusetts (required) Minimum 2 years of nursing experience (home health preferred) Strong clinical assessment and documentation skills Able to work independently with strong time management Reliable transportation for home visits Comfortable using electronic health record systems Preferred Qualifications Associate or Bachelor’s Degree in Nursing Multilingual abilities a plus (Spanish, Haitian Creole, etc.) Experience with home health or community-based services Benefits Flexible scheduling Competitive per-visit pay - $55 per visit Supportive and collaborative work environment Health and dental insurance options (for qualifying contractors) Join our dedicated team and help patients thrive at home.
