Medix is currently seeking a dedicated clinician for a travel opening at our client's facility. Responsibilities: JOB SUMMARY -Responsible for providing comprehensive assessment and nutrition treatment and coordination of 24-hour care for [Confidential] participants as part of the interdisciplinary team. Plans, organizes, and conducts a comprehensive program of individual and group nutrition services, including the provision of the nutrition needs of the participant, clinical guidance, nutrition education, and counseling to meet normal and therapeutic needs.KEY RESPONSIBILITIESPerforms initial face-to-face assessment and evaluates the nutritional status of individual participants utilizing nutritional guidelines, professional knowledge, and interviewing skills to provide quality care. Performs annual assessments and reassessments as requiredEvaluate participants’ physical condition and calculate therapeutic diets to meet the specific requirements of individuals. Makes recommendations to the team on the plan of care and status of participant progressProvides clinical recommendations and creates orders that will generate authorizations to reflect the individualized needs of each participant under the approval of the Primary Care ProviderMaintains an up-to-date file of all participants at assigned locations to include their diet order, modifications, allergies, restrictions, and likes/dislikesEstablishes nutritional plans on assessment of participant’s condition, monitors participant’s progress, and recommends/revises changes in nutritional plan for effective participant careDevelops and implements the nutrition care plan to promote, restore, or maintain the participant’s optimum nutrition status, ensuring integration into the interdisciplinary team’s plan of care. Monitors effectiveness and outcomes regularly and keeps the team informed as to participant progress and level of independenceDocuments change in the participant’s condition and detail care provided by completing all required [Confidential] documentation and ensuring compliance with Agency standards and policiesMonitors participant progress and outcomes through assessment of care provided, including the need for revision to the treatment plan. Coordinates with other members of the interdisciplinary team to ensure that the comprehensive plan of care is followed and revised as neededProvides nutrition counseling to participants and/or caregivers utilizing educational materials, modified diets, and teaching aids to ensure participants' understanding of the overall plan of care and its implicationsWorks with the Regional Medical Director to determine the nutritional adequacy of menus and the caloric and nutritional needs for the participant populationWorks with food service vendor to develop menus adequate to meet the caloric and nutritional needs of participantsActs as a liaison with other community agencies to gather and disseminate information on nutritional trends and changesConducts Dining/Meal Observation as neededManages stock of regular food supplies for breakfast and snacks and places orders as neededManages emergency food supply stock ensuring the [Confidential] Center maintains no less than a24-hour supply of food and waterDevelops a 4-week meal plan for the [Confidential] Center in conjunction with the catering vendorMonitors recorded refrigerator/freezer temperatures and recorded food temperatures to ensure safety and complianceEvaluates the need for home-delivered meals and maintains an up-to-date working file of participants receiving home delivered meals.Evaluates the need for nutritional supplements and maintains an up-to-date working file.Instructs participants/caregivers/family members and other staff members in proper use. The education of staff members will include but is not limited to providing in service training as needed and evaluating and documenting the competency of staff where appropriateWorks with the Recreation Therapist/Activities Coordinator and center staff to create activities, diet groups, programs, cooking demos, and educational activities Requirements •Bachelor's Degree from an accredited Dietetics programAbility to establish rapport with participants and staffA minimum of one (1) years’ experience in clinical nutritionMust have one (1) year of experience with the frail or elderly population Benefits Paid Sick Leave (Medix provides paid sick leave according to state and local sick leave ordinances).
