Job Description & RequirementsPRIDE Health is seeking a local contract Clinic Physical Therapist for a local contract job in Renton, Washington.Job DetailsJob Name: Travel Physical Therapist – Home HealthLocation: Renton, WA 98057Duration: 3 months ContractPayrate: $54–$56/hrSchedule: Monday – Friday | 8:00 AM – 4:30 PMWeekly Hours: 36–40 hoursTravel/Floating: 100% – must visit patients across South King County and may float to nearby LHC agenciesJob ResponsibilitiesEvaluate patients' functional status, body systems, and physical therapy needs per CMS documentationDevelop and implement individualized PT plans of care in coordination with physicians and healthcare teamObserve, monitor, and report patient responses and progress to supervisors or physiciansTreat patients to relieve pain, restore function, and maintain maximum functional independence using evidence-based techniquesEducate patients, caregivers, and healthcare team members on plan of care, functional limitations, and progress toward goalsComplete evaluations, progress notes, and discharge summaries according to LHC policies and state regulationsSupervise PTAs and home health aides as permitted by state regulations and agency policyParticipate in staff meetings, in-service education, and professional developmentProvide schedule flexibility, including potential weekend rotation or holiday coverage, to meet patient care needsAdhere to LHC Therapy Standards of Practice and maintain licensure and clinical knowledge up to datePerform all other duties as assignedRequirementsCurrent Physical Therapy licensure in Washington or a compact license granting practice privileges in WACurrent CPR certificationDriver's license, vehicle insurance, and access to a reliable vehicleBachelor's Degree in Physical TherapyStrong communication, clinical judgment, and patient care skillsNice to HaveHome Care/Home Health experience (HCHB)Experience with documentation and patient care in a home health settingBenefitsWeekly payHoliday PayGuaranteed Hours401k retirement planCancellation protectionReferral bonusMedical benefitsDental benefitsVision benefitsEqual Employment OpportunityPride-Health is an equal opportunity employer.
