Join us in making a positive impact on our residents' lives!Job Summary: The Physical Therapist shall screen, evaluate, re-evaluate, discharge plan, and make revisions in any established plan of care as supervised by the physician.Responsibilities/Requirements:* Screen all residents in facility on a quarterly basis, to identify those who have experienced declines in their functional abilities and determine if Physical Therapy intervention is indicated.* Evaluate residents per MD order and establish appropriate Treatment Plans and Goals that meet the specific needs of the individual.* Reassess patients and caseload at least every 30 days to determine the need for continued services or discontinuation of services based on patient progress. Carry out treatment plan as indicated.* Supervise treatment implementation carried out by Licensed Physical Therapy Assistants and determine appropriateness, as well as modify treatment plan as indicated.* Provide thorough discharge planning that will allow the patient to function at a maximal level of independence in the least restrictive environment.* Educate staff, family, and care givers as indicated.* Identify needs for program development and implement programs according to company policies and procedures.* Identify equipment needs of Physical Therapy Dept. and order equipment according to company and facility policy.* Complete all required documentation according to guidelines specified in Documentation Manual.* Coordinate in-services for facility staff as need is determined, to ensure safe and effective carry over of Maintenance/Restorative Programs and overall call of the residents.* Attend all facility clinical required meetings (Medicare, Care Plans, etc.).* Promote profession within facility, including assisting with coordinating special events i.e. National Physical Therapy Month.* Attend Continuing Education courses specific to current position.* Maintain professionalism and always follow all national/state associations regulations.* Supervise student affiliates as directed.* Assist employees in the absence of Clinical Manager.
