Job Description & RequirementsRN Case ManagerStartDate: 6/15/2026 Pay Rate: $1800.00 - $2700.00POSITION SUMMARY – RN Case ManagerPOSITION DUTIES – Coordinate patient care from admission through discharge, ensuring that treatments, services, and resources align with each patient’s medical needs and recovery goals. Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning.Minimum Required Qualifications –FL/compact RN licenseBLS2+ years minimum of acute care hospital experience in specialtyAcute hospital experience in specialty during the past year with no more than a 90-day breakMeditech & Midas experiencePREFERRED QUALIFICATIONS – Complex Discharge Planning ExperienceLENGTH OF ASSIGNMENT – 13 weeksSHIFT / HOURS PER WEEK – 8:30a - 5p. Every other weekend.SYSTEMS – Meditech, MidasSTART DATE – ASAPFacility LocationA beautiful 150 acre Botanical Garden represents Florida’s extensive native and tropical plants and has amazed all those who go visit the garden.
