We are a medium sized company with a family atmosphere.The Nurse Practitioner will provide services to patients in an office location. They will participate as an integral member of the healthcare team in a dynamic practice with physicians who value the role of the APRN.Essential FunctionProvide longitudinal and urgent care to patients in an office locationDevelop an adult and/or geriatric based patient panelEducate the patients on benefits of wellness visits vs. physicalsCreate a care plan for the patientUtilize EHR, Epic to update patient chartEngages patients and their families or caregivers in understanding, setting and monitoring self-management care plans, and documents individualized care plans and care coordination in the patient chart.Analyze patients medical history, medication allergies, physical condition and examination to determine best careDiagnose medical conditionPrescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.Directs and coordinates the patient care activities of nursing and support staff as required.Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and/or infection control standardsComplete Urgent Care and Annual Wellness VisitsEducation and ExperienceAPRNThe Advanced Practice Registered Nurse is masters or doctorate preparedCertified Nationally as a Nurse Practitioner through AANP or ANCC.Minimum of 2 years experience as a APRN requiredThe Nurse Practitioner must have possession of a valid (Connecticut) license as a Registered Nurse and an Advanced Practice Registered Nurse, and be certified as a Nurse Practitioner by an approved certifying body.DEA, and Ct controlled substance licenseExperience working with EHRPAPhysician Assistant CertificationState LicenseMinimum of 2 years experience as a PA requiredExperience working with EHR#APRNCT #NursePractitioner #AdvancedPracticeRegisteredNurse #APRN #PA #PhysicianAssistantEqual Opportunity Employer This employer is required to notify all applicants of their rights pursuant to federal employment laws.
