Senior Case Manager PSH - Rochester, NY
Volunteers of America - Upstate New York is seeking an energetic human services professional to join our housing team. If you're passionate about advocating for low-income and vulnerable people, delivering effective social services, and connecting with community partners we want to hear from you! The Senior Case Manager is responsible for overseeing program activities, providing support services to homeless individuals with one or more disabilities and their families to secure and maintain stable housing and household income.
Key Responsibilities
Deliver comprehensive housing and clinical case management services in alignment with VOA policies, HUD regulations, and community standards
Conduct intakes, biopsychosocial assessments, and develop individualized service plans
Provide individual and group life skills training, crisis intervention, and ongoing support services
Manage referrals, determine program eligibility, and coordinate appropriate placements
Oversee rental assistance processes while integrating supportive services to promote long-term housing stability
Manage program rent rolls, including calculating tenant rent portions, ensuring accuracy of rent determinations, and maintaining up-to-date records
Coordinate and oversee rent collection processes, including participant education on rent obligations, tracking payments, addressing arrears, and implementing appropriate follow-up actions in alignment with program policies
Conduct housing inspections and ensure compliance with Housing Quality Standards (HQS)
Build and maintain strong partnerships with landlords, healthcare providers, and community agencies
Utilize evidence-based practices including motivational interviewing, crisis de-escalation, and psychoeducation
Maintain accurate, timely, and compliant documentation (HIPAA, HUD, and agency requirements)
Lead case conferences and interdisciplinary team collaboration
Provide day-to-day guidance and support to case management staff, including assisting with complex cases, service planning, and problem-solving to ensure quality service delivery
Provide mentorship, training, and clinical guidance to staff as requested by supervisor
Audit case files regularly to ensure quality, compliance, and performance standards are met
Prepare program reports, outcomes tracking, and documentation as required
Coordinate caseload coverage and support program operations as needed
EXPERIENCE AND EDUCATION QUALIFICATIONS
Bachelor's degree in human services or related field, or equivalent experience
Minimum of 5 years of relevant experience
Valid NY State Driver's License and good driving record
Availability to work occasional evenings and weekends, and respond to emergencies as necessary
Strong organizational skills
Working knowledge of MS Office products
