Long-Term Care Pharmacist

A-Line Staffing is now hiring a Full-Time, On-Site, Long Term Care Pharmacist in Horsham, PA! Job Summary We are seeking a licensed Pharmacist to join our team in Horsham, PA for a second-shift position. The ideal candidate will have Long-Term Care (LTC) experience and familiarity with the AS/400 operating system. This role is responsible for ensuring safe, effective, and compliant medication dispensing while delivering high-quality patient care and consultation. Long Term Care Pharmacist Compensation The pay for this position is $66.70/hr (INCLUDES SHIFT DIFFERENTIAL) Benefits are available to full-time employees after 90 days of employment A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates Long Term Care Pharmacist Highlights This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs The required availability for this position 3:00 PM – 11:30 PM, alternating weekends Long Term Care Pharmacist Responsibilities · Accurately accept, review, and dispense prescription medications · Monitor patient medication compliance and identify overuse or misuse patterns · Ensure all pharmaceutical practices comply with applicable laws and regulations · Evaluate prescriptions for appropriateness, safety, and effectiveness · Maintain accurate, confidential patient records and pharmacy documentation · Provide patient counseling on prescription and OTC medications, including dosage, storage, side effects, and interactions · Educate patients on use of medical devices (e.g., blood pressure monitors, glucose meters) · Offer clinical services such as smoking cessation, blood pressure checks, and cholesterol management · Compound medications and prepare sterile solutions following proper protocols · Supervise pharmacy technicians, clerks, and support staff · Maintain appropriate inventory levels of pharmaceuticals and medical supplies · Identify and resolve drug-related problems in collaboration with physicians and healthcare teams · Work collaboratively with physicians, nurses, pharmacists, and interns Long Term Care Pharmacist Requirements · Doctor of Pharmacy (PharmD) required · Active Pharmacist license in the state of Pennsylvania · Long-Term Care (LTC) experience preferred · Experience with AS/400 operating system preferred · Strong attention to detail and commitment to patient safety · Excellent communication and interpersonal skills · If you think this Long-Term Care Pharmacist position is a good fit for you, please reach out to me—feel free to call, e-mail, or apply to this posting!
