Licensed Vocational Nurse Rehab

Methodist Hospital, part of SA Health and a leading healthcare provider in San Antonio, is seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) for our Rehab unit within the Healthcare & Medical Services department. As an LVN Rehab nurse, you will provide direct, patient-centered nursing care to adult and geriatric patients recovering from illness, injury, and surgery, supporting their return to optimal function and independence. In collaboration with Registered Nurses, physicians, therapists, and other members of the interdisciplinary team, you will assist with patient assessments, administer medications and treatments as ordered, monitor vital signs and clinical status, and promptly report changes in condition. You will support rehabilitation plans of care by reinforcing therapy goals, assisting with mobility and activities of daily living, and educating patients and families on safety, self-care, and ongoing recovery needs. Your role includes accurate and timely documentation in the electronic medical record, adherence to infection prevention and hospital safety protocols, and participation in care conferences and handoffs to ensure continuity of care.
