In this role, you serve as a vital link between patients managing long-term health conditions and their clinical care teams. You will provide essential support by conducting monthly outreach, helping patients follow their care plans, and monitoring health data remotely. This is an ideal role for a clinician who wants to make a measurable impact on patient outcomes while enjoying the flexibility of a remote work environment.What Success Looks LikeYour primary goal is to ensure patients feel supported, informed, and engaged in their treatment plans. Success in this role is measured by your ability to build trust with patients, accurately document health interactions, and provide clear communication to the broader care team.Daily: You will manage a schedule of outreach calls to patients, conduct health risk assessments, document clinical interactions in our EHR system, and assist with Remote Patient Monitoring (RPM) tasks. You will act as a primary point of contact for patient questions and medication adherence supportOver Time: You will become a subject matter expert in our care coordination workflows, helping to improve patient engagement metrics and ensuring that quality-of-care standards are consistently met. You will grow your ability to manage a patient panel effectively, anticipating clinical needs before they become urgent issuesRequirementsMust-HavesActive, unencumbered CMA or RMA certificationCurrent Basic Life Support (BLS) certificationProven experience in a clinical or care coordination settingStrong proficiency in EHR systems and virtual documentationExcellent verbal and written communication skills with the ability to explain complex health information clearlyA dedicated, HIPAA-compliant, and distraction-free home office environment with high-speed internetAbility to work independently and manage a structured daily workflow without direct supervisionNice-to-HavesPrior experience specifically in Chronic Care Management (CCM) or Transitional Care ManagementFamiliarity with navigating community resources to support patient social determinants of healthExperience working with quality metrics and CMS compliance standardsDemonstrated success in high-volume, fast-paced remote healthcare environmentsBenefitsThe pay range for this position is $17.00 - $20.00 per hour.We believe in supporting our team members both inside and outside of the office.
