Main Function
Conduct Medicare Annual Wellness Visits in accordance with CMS guidelines under provider supervision
Collect and document comprehensive patient histories, including:
Medical, surgical, family, and social history
Medication reconciliation (prescription, OTC, supplements)
Allergies and immunization status
Perform and document required screenings:
Depression screening (PHQ-2/PHQ-9)
Cognitive impairment screening
Fall risk assessment
Health risk assessment (HRA)
Tobacco, alcohol, and substance use screening
Record vital signs, height, weight, BMI, and other relevant clinical data
Identify preventive care gaps and assist with development of personalized prevention plans
Provide patient education on preventive services, chronic disease management, and lifestyle modifications
Ensure accurate and timely documentation in the electronic health record (EHR)
Assist providers with outpatient clinical workflows as needed
Prepare patients for provider visits and procedures
Administer injections, immunizations, and medications per scope of practice and provider orders
Perform basic clinical procedures consistent with LPN licensure
Coordinate referrals, diagnostic testing, and follow-up appointments
Communicate test results and care instructions to patients per protocol
Maintain exam room cleanliness and stock supplies
Adhere to infection control, safety, and HIPAA regulations
Support quality initiatives, population health programs, and preventive care metrics
Collaborate with providers, care coordinators, and administrative staff
Demonstrate compassionate, patient-centered care and professionalism
Additional responsibilities required
Floating to other GFP offices may be required
Education
Current, unrestricted Licensed Practical Nurse (LPN) license (state-specific)
Graduation from an accredited practical nursing program
Knowledge of outpatient clinical workflows
Basic computer skills and experience with EHR systems
Strong communication, organizational, and documentation skills
Experience
Experience conducting Medicare Annual Wellness Visits
Familiarity with CMS preventive care guidelines
Outpatient primary care or internal medicine experience
Bilingual skills preferred
