Our centers of excellence include Baptist MD Anderson Cancer Center, Baptist Heart Hospital, Baptist Neurological Institute and Wolfson Children's Hospital.This role is responsible for driving outcomes, procedures, and processes of the system-wide Case Management program the adult hospitals and reports to the Executive Director of Care Coordination. This leader partners with the Executive Director of Care Coordination to provide strategic and operational oversight to ensure high-quality, efficient, and patient-centered care coordination throughout the inpatient setting. This role is also responsible for direct oversight and mentoring of the Managers of Inpatient Case Management and driving accountability as it pertains to leadership supervision, operational excellence, throughput initiatives, length of stay, readmission prevention, and capacity management. The role partners closely with Social Services leadership developing and implementing team-based models of care resulting in positive patient outcomes and operational success. This role ensures patients and families have seamless access to care across the care continuum and will serve as a strategic partner to hospital and health system leadershipPrimary Location:Downtown - 841 Building
