Job Description & Requirements Specialty: Home Health Discipline: RN Start Date: 05/25/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Local Contract Position Overview The Hospice RN Case Manager provides skilled nursing and case management services to hospice patients, focusing on ongoing care coordination rather than admissions. This role supports patients and families through comprehensive hospice management, interdisciplinary collaboration, and compassionate end of life care. Schedule & Work Details Coverage Area: Lewis County Schedule: Monday through Friday, 8:00 AM to 4:30 PM Weekly Hour Guarantee: 40 hours On Call/Weekend Requirement: On an as needed basis when on call or weekend staff call out Mileage Reimbursement: Yes, reimbursed between patient visits from first patient to last patient Pay Rate: $51 to $53 per hour Key Responsibilities Manage a hospice caseload as an RN Case Manager Complete interdisciplinary group (IDG) documentation and care coordination Perform hospice recertifications, live discharges, and death visits Ongoing assessment, symptom management, and care plan updates Coordinate care with physicians, social workers, chaplains, aides, and the hospice interdisciplinary team Provide patient and family education related to disease progression, medications, and end of life expectations Ensure timely, accurate documentation in compliance with hospice regulations and agency standards Maintain compliance with state, federal, and hospice specific guidelines No admissions required Required Qualifications Active Registered Nurse (RN) license Previous hospice nursing experience Strong case management and clinical assessment skills Ability to work independently in the field Valid driver's license and reliable transportation Preferred Qualifications Experience with Homecare Homebase (HCHB) Strong organization, time management, and communication skills Compassionate, patient centered approach to end of life care About PRIDE Health PRIDE Health is the minority-owned healthcare recruitment division of Pride Global—an integrated human capital solutions and advisory firm.
