**Job Title: Home Health Physical Therapist • Days**Seeking a dedicated Physical Therapist for home health care in Methuen, MA, and Nashua, NH.**Responsibilities:**• Provide quality physical therapy to home health patients.• Document using HCHB and OASIS, ensuring same• day submission.• Manage 20• 25 cases weekly, achieving 26• 28 productivity points.• Attend weekly in• person office meetings.• Offer flexible care beyond standard hours (8am • 5pm).**Requirements:**• Licensed Physical Therapist in MA and NH.• 2+ years of home health experience with HCHB/OASIS.• BLS certification.• 40• hour workweek with flexibility to float.• Reside within 30 minutes of the territory or secure nearby housing.**Details:**• 13• week contract, possible extension.• No on• call or overtime.• Mileage reimbursement.• Any scrub color.
