Role Description This is a full-time role as a Home Health Physical Therapist based in South Dallas, TX. The role involves assessing patients' physical therapy needs, developing and implementing tailored treatment plans, and providing hands-on care in the home setting. You will work collaboratively with multidisciplinary teams, monitor patient progress, and provide education to patients and their families to promote optimal recovery and independence. Qualifications Expertise in Physical Therapy and Rehabilitation techniques for diverse patient needs Strong knowledge of Home Care best practices Proven ability to deliver high-quality Patient Care with empathy and professionalism Experience working in home health settings or providing individualized care, preferred Valid Physical Therapist license in the state of Texas Effective communication and collaboration skills with patients, families, and care teams Proficiency in documentation and adherence to patient care protocols Reliable transportation and willingness to travel within the South Dallas area
