Role & Responsibilities: Provide comprehensive care, including H&Ps, admissions, progress evaluations, transfers, and discharges, for patients in post-acute settings. Collaborate with an interdisciplinary team to ensure a seamless transition of short-term rehab patients to the most appropriate level of care (home, assisted living, memory care, etc.). Work closely with hospice, palliative care, and home care agencies to ensure secure patient transitions.
