Job Brief1 OpeningPay Range: SOCIAL WORKER - $31.16 to $49.86Job SummaryThe EmPATH Social Worker is a member of a multi-disciplinary team responsible for providing social work-related services to the adult population in the crisis stabilization unit. The Social Worker coordinates care of the patient by interfacing regularly with various members of the patient’s multi-disciplinary treatment team—Emergency Department providers, psychiatrist, program manager, clinical RNs and staff. The Social Worker provides assessment and case management services, including biopsychosocial and behavioral health assessments, treatment/discharge planning, group, individual and family therapy. They provide assistance to both patients and families to meet related social and emotional needs for psychiatric condition, treatment and recovery.Licensure And Certification Current BLS provider card Current Associate Clinical Social Worker (ACSW) through BBS Current CA LCSW preferred Lanterman-Petris-Short (LPS) Certificate required within 60 days of employment. Education Master’s Degree in Social Work Experience One year of hospital and/or mental health social work experience required. Care management experience preferred. Preferred 2-3 years in Behavioral Health related field Knowledge And Skills Strong verbal and written communication skills Strong organizational skills Problem solving skills Patient-focused Ability to navigate and connect successfully with outside provider agencies (Health Plans, DHHS). Physical Demands – Clerical/Administrative Non-Patient Care Frequent sitting and standing/walking with frequent position change. Continuous use of bilateral upper extremities in fine motor activities requiring fingering, grasping, and forward reaching between waist and chest level. Occasional/intermittent reaching at or above shoulder level. Occasional/intermittent bending, squatting, kneeling, pushing/pulling, twisting, and climbing. Occasional/intermittent lifting and carrying objects/equipment weighing up to 25 pounds.
