This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.Demonstrate proficiency in practice based on conscious and deliberate planning. Self-directed in goal setting for managing complex client situations. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines. Influences outcomes by collaborating with members of the interdisciplinary team. Functions or Scope of Assigned Duties: The Community Living Center (CLC) Registered Nurse (RN) is responsible for providing competent, evidence-based care to residents with oversight of licensed vocational nurses/nursing assistants as appropriate to the setting. The nurse will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and enhance outcomes utilization. This nurse integrates knowledge, skills, abilities, and judgment and is self-directed in goal setting for managing complex situations. Validates understanding of the rationale for care processes and decisions. Questions clinical practices for the purpose of providing evidence-based care. The nurse will demonstrate the ability to cope with and manage competing priorities. The nurse demonstrates competence in practice and decision-making, deliberate planning, and critical thinking skills. The CLC RN collaborates with the Minimum Data Set (MDS) Coordinator to ensure accuracy of the Resource Utilization Group (RUG) score. The RN is a Direct Care Nurse/Clinical Staff Nurse who provides care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process. This includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, health counseling, provision of care supportive to or restorative of life and well-being; and executing medial regimens including administering medications and treatments prescribed by a licensed or otherwise legally authorized provider.Provides age - appropriate, culturally, ethnically, and spiritually sensitive physical and mental care support.Adopts a lifespan perspective that appreciates the meaning and impact of ageism, poverty, and disability.Educates older adults and families about chronic illness and mental health care and treatment options.Assist in the coordination of care among caregivers.Participates in quality care improvement activities.Participates in the Shared Decision Making Team and promoting a shared governance environment on the unit.Administer medication and procedures per established policies and guidelines.The RN is included in a rotational charge nurse coverage which includes, but is not limited to:Directs RN team leads and other team members in provision of patient care.Makes out staff neighborhood daily assignments for all three shifts at least 24 hours in advance, ensuring consistency with staffing.Assists in assignments of patient care, decision-making, role modeling, mentoring and educating staff throughout the work day.In absence of the Patient Care Coordinator: Attends Interdisciplinary Team rounds, huddles and meetings.Rounds on team members, implements watchlists and other patient care needs that arise.Leads and/or participates in shift to shift huddles, ensuring all team members are informed of any issues or changes.Acts as point of contact for medical providers needing tasks completed in an urgent and timely manner.* Other duties as assigned within scope of practice/licensureVA offers a comprehensive total rewards package: VA Nurse Total RewardsPay: Competitive salary, regular salary increases, potential for performance awardsPaid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VAInsurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)Licensure: 1 full and unrestricted license from any US State or territoryWork Schedule: Night Shift - 7:30pm-8:00am (6) and 11:30pm-8:00am (1) in bi-weekly pay period, rotating weekends and holidaysTelework: Not AvailableVirtual: This is not a virtual position.Relocation/Recruitment Incentives: Not AuthorizedEDRP Authorized: Former EDRP participants ineligible to apply for incentive.
