Holocaust Services Geriatric Case manager
This position is responsible for assessing individuals and couples capabilities to remain safely at home or assist them in developing alternate living arrangements. Assists in gaining access to needed medical, social, educational, psychological and other essential services. Assess clients’ strengths and limitations and develop a plan of care. Coordinate services, linkage to community resources and monitor overall service delivery.
