Patient Care Connect CoordinatorLocation: Torrance, CA 90505 Duration: 3+ months with the potential for extension based on business needs and/or possible conversion 8 AM to 5 PM, Monday Friday Pay Rate is $30 an hour Training will take place at Los Angeles, CA 90033, THEN will move to Torrance, CA 90505Position Summary:Clerical ResponsibilitiesGather patient demographics, insurance information, and referral documentation to accurately register new and returning patientsAct as a patient liaison and primary point of contact for patients and caregiversEnter patient information accurately into computer systems utilizing proper Care Connect functionsConfer with patients and third-party payers to ensure eligibility and authorization of servicesIdentify alternative financial sources for services and facilitate processing of applications and approvalsHandle patient account inquiriesMaintain accurate patient records within the electronic medical record (EMR) systemSchedule patient appointments in a timely manner based on physician requests and patient needs, including laboratories, diagnostic testing, and consultationsPatient Coordination / Customer ServiceGuide patients through the process by explaining procedures, providing directions, and answering questions regarding visitsCoordinate patient communication, appointment scheduling, inquiries, and care between patients and healthcare providersEnsure a smooth patient experience by addressing concerns and facilitating information flow within the healthcare systemUtilize courtesy, compassion, kindness, and honesty while interacting with patients, the public, and clinic personnelGreet patients in a courteous and professional mannerAssist patients with accessing and utilizing the online patient portal to view medical history and communicate with their healthcare teamProvide basic information regarding procedures, medications, and post-care instructionsEscort patients to other departments when needed and provide directions within the systemAssist patients in overcoming barriers to care, including coordination of translation services and medical financial assistanceAct as a communication hub between patients, healthcare providers, and other internal and external departmentsWork with hospital personnel to resolve issues and accommodate patient needs.Required Experience / EducationHigh school diploma or equivalent required12+ years of healthcare scheduling, patient access, or front office experience preferredStable onsite schedule with a collaborative clinic teamDirect impact on improving patient access and experienceMust-HavesExperience in healthcare or a medical office setting, particularly with schedulingBackground in high-volume patient scheduling or call center environmentsStrong customer service and patient communication skillsFamiliarity with EMR/EHR systemsAbility to multitask effectively in a fast-paced clinic environmentProfessional and polished appearanceNice-to-HavesBilingual (Spanish highly preferred)Orthopedics or specialty clinic experienceInsurance verification or referrals knowledgePrior access center or patient navigation experience
