Registered Nurse

Job responsibilities include but are not limited to the following:Coordinate total client care by conducting comprehensive health and psycho-social evaluation, monitoring the client's condition, promoting sound preventative practices, and coordinating services and training activities.Consult with the attending physician concerning alterations of client care plans. Notify the appropriate supervisor and makes changes as appropriate.Prepare clinical and progress notes to be submitted on a weekly basis.Participate in case conferences. Discuss client concerns with the supervisor and seek appropriate resolutions.Participate in the client's discharge planning process.Cooperate with other agencies providing nursing or related services to provide continuity of care and to implement a comprehensive care plan.Continually strive to improve nursing care skills by attending in-service training, formal education, attendance at workshops, conferences, active participation in professional and related organizations and individual research and reading.Maintain an on-going knowledge of current drug therapy.Registered Nurses must possess the following credentials in order to be considered for employment.Active CT Registered Nurse in good standing.At least one year of nursing experience, preferably in a home care setting.Experience with trach, vent, and g-tube clients preferred.Flexible availability during the afternoon, evenings, and weekends.Valid CT drivers license and reliable transportation.CPR certification or willingness to obtain.
