Come join the team that makes this happen.Applicants MUST apply for position(s) by submitting a separate application for each individual job posting number they are interested in being considered for.FTE1Scheduled Bi-Weekly Hours80Work ShiftDay: 8 hoursJob DescriptionJob DescriptionInventory currently available population health tools and data sets in EpicIdentify gaps in ECH's population health tools and data sets in EpicMap current population health activities across ECH departmentsDevelop standard population health workflowsUsing data available in Epic and/or other sources, identify the target populations that could most benefit from population health interventionIn collaboration with Quality, identify additional data which may need to be collected in Epic and/or other sources in order to track potential health disparities which may inform the population health interventionsIdentify champion physician(s) and collaborating departments within ECHContribute to the development of a comprehensive population health strategy for ECHD over the next 1-3 yearsQualificationsBachelor's Degree in Public Health, Health Administration, Social Work or a related field required.Must have at least 3 years' experience managing increasingly complex projects in a healthcare environmentDemonstrated knowledge of population health management concepts and strategiesAbility to work effectively and efficiently toward goals in a complex, diverse environment with multiple and changing demandsMust have an outstanding grasp of information technology concepts and processesDemonstrated communication effectiveness and good project management disciplineStrong data analysis, research and program evaluation skillsAbility to foster trusting relationships with colleagues and clientsLicense/Certification/Registration RequirementsAges of Patients ServedThis position does not generally require contact with patients, but may communicate with patients of all age categories.Salary Range:$66.85 - $100.27 USD HourThe Physical Requirements and Working Conditions of this job are available.
