Job RequirementsPOSITION SUMMARYThe Clinical Charge Nurse of Home Care demonstrate leadership through the assessment, planning,implementation, and evaluation of patient care according to the standards of professional nursingpractice. The Clinical Charge Nurse acts as a resource to the Assistant Nurse Manager (ANM) andNurse Manager (NM) as appropriate for precepting, scheduling, team meetings, and clinicalimplementations as directed.PRIMARY ACCOUNTABILITES1. Appropriately delegates and supervises all patient care activities within the Healthcare setting.2. Effectively communicates with other members of the healthcare team and promotes effectiveteam functioning.3. Helps to monitor the effectiveness of existing processes and policies towards improving qualityoutcomes; actively participates on quality projects.4. Recognizes and resolves safety issues and potential unsafe situations within approved policies,procedures and protocol.5. Actively participates in the development and attainment of department team goals.6. Uses critical thinking, prudent judgment and direction to staff in nursing assessment, diagnosis,planning, interventions and evaluation of outcomes, and adjusts plan of care as needed.7. Facilitates and leads effective interdisciplinary care team meetings; serves in a coaching,mentor or instructive role to nurse interns, students and peers, etc.8. Effectively leads shift and post-event huddles when appropriate9. Participates and provides feedback to manager for associate interviews, check-ins andperformance management.10. Demonstrates effective conflict resolution skills, demonstrates dependability, reliability andflexibility.11. Adds positively to creating and sustaining a healthy work environment and staff retention,oversees orientation of new clinical staff.12. Serves as a resource to address points of escalation with patients, families and staff.Work ExperienceMINIMUM QUALIFICATIONSEducation: Associate's degree in Nursing (ASN), or Nursing DiplomaWork Experience: NoneLicensure:o Registered Nurse (RN) licensure in the State of Florida or endorsement.o Valid Driver's License and clean Motor Vehicle Report (MVR) and maintained.* Certification: American Heart Association Basic Life Support (AHA BLS) Healthcare ProviderCompletion Card prior to start date and maintained.* Background Check: Must satisfy Florida Agency for Health Care Administration (ACHA) Leveltwo (II) background screening.* Skills/Knowledge/Abilities:o Must have reliable transportation to care for clients in the home-setting.o Must be able to speak and understand written and verbal instructions in the Englishlanguage.o Must be able to participate in AOC rotation, on call rotation; weekend/evening rotationo Strong critical thinking and decision-making skills.o Proficient computing skills include use of MS Office Suite and other softwareapplications and online resources.PREFFERED QUALIFICATIONS* Education: Bachelor's degree in Nursing (BSN).* Work Experience: Two (2) years of Acute Care, Medical-Surgical, or Home Health clinicalexperience.PHYSICAL REQUIREMENTS* Physically agile; may require walking, standing, running, bending, stooping, kneeling, climbing,or crouching frequently.* May occasionally include lifting or moving objects up to 50 pounds, with or without assistance.* May require moving in different positions to accomplish tasks in various environments includingtight and confined spaces, including reaching out and above shoulders, or overhead.* May lift, turn, pull, push, or ambulate people and/or equipment with or without assistance up to90% of the assigned shift.* May be exposed to inside and outside environments with varied temperatures, air quality,lighting, and/or low to loud noise.* May require working with biohazards such as blood/body fluids and airborne particles followingOSHA and CDC guidelines.Communicating with others to exchange information.Visual acuity and hand-eye coordination to perform tasks.May require travel to various facilities within and beyond county perimeter; may require use ofpersonal vehicle.BenefitsABOUT HEALTH FIRSTAt Health First, diversity and inclusion are essential for our continued growth and evolution.
