You will report to the Clinical Manager.We offer A $10K SIGN-ON BONUS, for this opportunity!As a Home Health Physical Therapist, you will:Confer with physician and clinical team members to obtain patient information and help develop, implement and revise the therapy treatment program.Plan and administer prescribed skilled physical therapy treatment and training for patients suffering from various injuries, illnesses and functional disabilities to attain highest level of physical functionSubmit evaluation, treatment plans, progress reports and discharge summary to the supervisor and care management staffProvide Physical Therapy Assistants and Home Health Aide staff with written instructions and care plan that reflects current plan of care as related to therapyOversee and evaluate Physical Therapy Assistant and Home Health Aide staff performance, including monitoring the appropriate completion of documentationReport patient progress to the patient's physician, Clinical Manager, staff, patient and familyUse your skills to make an impactRequired QualificationsCurrent and unrestricted Physical Therapy license in the state of North CarolinaA valid driver's license, auto insurance, and reliable transportationCurrent CPR certificationPreferred Qualifications* Home Health experiencePay Range$59.00 - $83.00 pay per visit/unit$93,000 - $128,000 per year base payAdditional InformationDriving StatementYou will be a part of Humana's driver safety program and therefore will need a valid state driver's license and proof of personal vehicle liability insurance with at least 100/300/100 limits.TB StatementYou will be patient facing, therefore apart of Humana's Tuberculosis (TB) screening program. If selected for this role, we will require you to undergo TB screening.Scheduled Weekly Hours40Pay RangeThe compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting.
