McGregor PACEMcGregor PACE is a Program of All-inclusive Care for the Elderly (PACE), which provides wrap-around services that address the medical, functional, social, and personal healthcare needs of frail seniors living in our community.We are seeking an Adult/Geriatric Nurse Practitioner to work in collaboration with our Interdisciplinary team in a supportive practice environment. The position requires an energetic, risk-tolerant applicant who enjoys working in a team designed to support elders at risk for nursing home placement- and help them remain in their homesHours are Monday through Friday, day shift only covering our Brooklyn and Elyria office locations (west side).Essential Job Functions:Works as primary care provider with on-site primary care physician collaboration to deliver comprehensive health services to their panel of participants, including assessment, diagnosis, clinical decision making, treatment and prescribing.Directly manage acute and chronic illness, promote wellness and preventative care, and follow their patients into various outpatient care settings (including skilled and long-term nursing facilities, assisted living facilities, group homes, and participant homes) in collaboration with the Interdisciplinary Team (IDT).Interdisciplinary support includes on-site integrated collaboration with nursing, social work, physical/occupational/speech therapy, transportation, dietary services, and mental health services.Together with the IDT, provides on-going assessments with a focus on physical, mental and behavioral needs to create and implement a plan of comprehensive care.Serves as a clinical resource for nursing staff and preceptor for clinical orientation. Opportunities for precepting students (not required).Assists in program and participant outcome data collection and analysis; participates in performance improvement initiatives.Is responsible for identifying and working within the scope of practice permitted by Ohio and Federal laws and regulations, the professional code of ethics, and professional practice standardsResponsible for achieving continuing education and other requirements for licensure, certification and prescriptive authorityParticipates in clinical on-call rotation (from home)Qualifications:Masters prepared nurse with certification as a Family Nurse Practitioner or Adult/Gerontology Nurse Practitioner holding active, unrestricted license in the state of Ohio, required.Minimum of two years direct care experience working with frail, older adults in community, long term, skilled care, or hospital, requiredMinimum of two years advanced practice nursing experience, requiredPrevious experience in interdisciplinary care setting, desirableBilingual candidates (English/Spanish) are encouraged to applyCPR (BLS) certification, requiredWe offer a COMPETITIVE SALARY with great health benefits in addition to Paid Time Off, Sick Time, 8 paid Holidays, a 403b matching Retirement Plan, Quality-Based Bonus pay, Health Savings Account and more!
