Team Lead for Community Support Team

The primary role of the Community Support Team Leader is to drive the delivery of this 24/7/365 service, utilizing a team approach, including provision of services that will include direct and indirect interventions. CST is provided in a variety of locations. This service consists of community-based mental health and substance use services, and structured rehabilitative interventions intended to increase and restore a beneficiary's ability to live successfully in the community. The team approach involves structured, face-to-face therapeutic interventions that assist in reestablishing the beneficiary's community roles related to the following life domains: emotional, behavioral, social, safety, housing, medical and health, educational, vocational, and legal. The individual's clinical needs are evidenced by the presence of a diagnosable mental illness, substance-related disorder as defined by the DSM-5 (or current version), or both with symptoms and effect documented in the comprehensive clinical assessment and the Person Centered Plan. CST shall provide "first responder' crisis response 24 hours a day, 7 days a week, 365 days a year to recipients of this service.
