The Physical Therapist develops individualized treatment plans designed to improve strength, balance, coordination, and overall physical function.This role involves working closely with patients, caregivers, and the interdisciplinary healthcare team to promote recovery and improve quality of life.QualificationsRequiredDoctor of Physical Therapy (DPT) or Master's degree in Physical TherapyCurrent Physical Therapist license in the State of ConnecticutCurrent BLS certificationStrong clinical assessment and treatment planning skills PreferredExperience in home health or rehabilitation settingsExperience working with geriatric or post-acute care patientsStrong documentation and patient education skills What You'll DoConduct comprehensive physical therapy evaluations for patientsDevelop individualized treatment plans based on patient needs and physician ordersProvide therapeutic exercises and mobility trainingMonitor patient progress and adjust treatment plans as neededEducate patients and caregivers on exercises and safety techniquesDocument patient care and treatment outcomes in the electronic health recordCollaborate with nurses, physicians, and other healthcare professionals to coordinate careEnsure compliance with clinical and regulatory standards CompensationSalary: $90,000 - $110,000 per yearBenefits & PerksAt Health Care Connectors, we believe in supporting our employees both professionally and personally.
