This role focuses on comprehensive assessments, care planning, clinical oversight, and coordination with an interdisciplinary team to ensure safe, high‑quality, and patient‑centered home health services.Schedule & Work Details:Weekly Hour Guarantee: 36 hoursSchedule: Monday through Friday, 8:00 AM to 4:30 PMOn Call and Weekend Requirement: Yes, participation requiredCoverage Area: South King CountyMileage Reimbursement: Yes, reimbursed between patient visits from first patient to last patientPay Rate: $51-$53 per hourKey Responsibilities:Initiate, develop, implement, and revise individualized plans of care in collaboration with physicians and interdisciplinary care team membersProvide skilled nursing services in accordance with the plan of care, scope of practice, and infection control standardsPerform comprehensive nursing assessments including OASIS assessments and medication reconciliation in an accurate and timely mannerSupervise and provide guidance to home health aides and licensed practical vocational nurses, including completion of required supervisory visits and task delegation per state and federal regulationsEnsure patients meet home health eligibility and medical necessity requirements based on payer guidelines and clinical evaluationAccurately and thoroughly document all patient visits in compliance with agency policy, payer requirements, and LHC documentation standardsCommunicate effectively with physicians, patients, families, schedulers, and agency staff to coordinate care, manage visit schedules, and address patient care needsParticipate in case conferences, performance improvement initiatives, and survey preparedness activitiesSupport team collaboration by assisting with orientation of new staff, serving as a preceptor when needed, and maintaining schedule flexibility to meet patient care demandsParticipate in weekend rotations, potential holiday coverage, and adhere to the agency’s utilization management modelRequired Qualifications:Current unencumbered Washington State Registered Nurse (RN) licenseCurrent BLS CPR certificationValid driver’s license, active vehicle insurance, and access to reliable transportation or public transitPreferred Qualifications:Prior home health nursing experienceExperience with Homecare Homebase (HCHB) documentation systemPride Global and its affiliates offers eligible employee’s comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance and hospital indemnity), 401(k)-retirement savings, life & disability insurance, an employee assistance program, legal support, auto, home insurance, pet insurance and employee discounts with preferred vendors.Equal Employment OpportunityPride Global and its affiliates are an equal opportunity employer.
