EDWP Structured Family Caregiving Program
Position Summary
The Structured Family Caregiver (SFC) Health Coach – LPN supports family caregivers enrolled in Georgia’s Elderly and Disabled Waiver Program (EDWP) by providing ongoing monitoring, education reinforcement, and caregiver coaching under the clinical supervision of a Registered Nurse (RN). This role focuses on caregiver support, condition monitoring, and care plan adherence, not hands‑on skilled nursing.
Key Responsibilities
Caregiver Coaching & Support
Serve as the primary point of contact for assigned Structured Family Caregivers
Reinforce caregiver training related to:
Activities of Daily Living (ADLs)
Safety and fall prevention
Chronic condition management strategies
Provide guidance to caregivers on maintaining compliance with the individualized care plan
Monitoring & Reporting
Conduct routine caregiver check‑ins (telephonic and/or in‑home as required)
Monitor participant status and identify changes in condition
Document observations and escalate concerns to the supervising RN promptly
Review caregiver logs and service documentation for completeness and accuracy
Care Coordination
Communicate regularly with the supervising RN regarding participant status
Support RN‑led care plan updates and reassessments
Coordinate with case managers and internal team members as directed
Compliance & Documentation
Maintain accurate documentation in accordance with EDWP, DCH, and provider policies
Participate in audits, quality assurance activities, and required trainings
Follow all delegation and supervision requirements under Georgia nursing law
Scope of Practice
This position does not include independent nursing assessments or care plan development
All clinical activities are performed under RN supervision in accordance with the Georgia Nurse Practice Act
Qualifications
Required Active Georgia LPN license in good standing
Minimum 1 year experience in home care, long‑term care, geriatrics, or community‑based services
Ability to work under RN supervision and follow delegated clinical protocols
Strong communication and caregiver‑education skills
Preferred
Experience with Medicaid waiver programs (EDWP, CCSP, SOURCE)
Familiarity with Structured Family Caregiving or caregiver support models
Bilingual abilities (preferred but not required)
Work Environment
Community‑based / home visits and remote work
Flexible scheduling may be available
Mileage reimbursement and benefits per company policy
Job Type: Full-time
Pay: $35.00 - $40.00 per hour
