UAS RN Care Manager Compensation • The pay for this position is $60.00 per hour • Benefits are available to full-time employees after 90 days of employment • A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates UAS RN Care Manager Highlights • This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs • Field-based role – candidates must reside in the Bronx or Brooklyn, NY area • The required availability for this position is Monday – Friday, standard daytime hours • Conduct 2–4 UAS assessments or reassessments per day in the field • Home and site visits required UAS RN Care Manager Responsibilities • Perform care management duties to assess and coordinate medical and support services for high-acuity and complex members • Conduct UAS assessments and develop personalized long-term care service plans • Collaborate with providers, specialists, and community resources to meet member care needs • Monitor member progress, changes in condition, and effectiveness of care plans • Educate members, families, and caregivers about treatment plans, services, and healthcare benefits • Coordinate services such as transportation, housing support, daily living assistance, and community resources • Maintain accurate documentation of care management activities in accordance with state and federal regulations • Serve as a liaison and advocate between members, families, physicians, and care providers • Participate in quality improvement initiatives and support care management teams • May assist with mentoring or precepting new clinical staff UAS RN Care Manager Requirements • Active New York Registered Nurse (RN) license • Minimum of 4 years of relevant RN experience • UAS assessment experience required • Ability to perform field visits and home assessments • Strong care coordination, clinical assessment, and care planning skills • Valid driver’s license (personal transportation preferred; public transportation considered) • High School Diploma or GED • Attendance is mandatory for the first 90 days UAS RN Care Manager Preferred Qualifications • Bachelor’s Degree in Nursing (BSN) • Experience in care management or managed care organizations (MCO) • Experience working with complex or high-acuity patient populations • Knowledge of community resources and long-term care coordination • Leadership experience mentoring or training clinical staff If you think this UAS RN Care Manager position is a good fit for you, please reach out to me – feel free to call, e-mail, or apply to this posting! .
