We're growing and need professional care managers!SeaStar Senior Services is a professional in-home care and care management company in Portland, Oregon and is looking for a professional Care Manager who wants to be part of a great company that is focused on providing quality, compassionate care to our clients.Position Details: This position is very part time to start - 3 to 5 hours - then will increase quickly as new clients are brought on board weekly. Requires flexible availability during business hours and participation in after-hours on-call rotation.DutiesPerform comprehensive assessments, gathering information about health history, family and social dynamics, natural support systems, and environmental concerns and needs.Create thorough care plans enlisting supports already in place and arranging access to other needed services.Implement care plan actions to reach goals established during assessment and care planning.Update assessments and care plans quarterly or as needed.Respond in timely manner to changes in client's condition or environmentDetermine housing needs and assist in placement for higher level of care or arranging resources for client safety to remain in their homeAttend care conferences and serve as liaison between facilities, family, and professional fiduciariesCoordinate all medical appointments, specialty follow up visits, procedures, and obtain needed medications.Obtain other needed professional services including legal, financial, care, and more.Provide or arrange for transportation to appointments and social outingsDocument all work performed on client's behalf with detailed notes and accurate time or provide monthly reports to responsible parties as desiredParticipate in weekly on-call rotation for evenings and weekends.Perform other related duties as assignedQualificationsRegistered Nurse License OR Master's in Social Work degree REQUIREDExperience working with seniors and adults with disabilitiesExcellent driving record and own carCPR certified or able to obtainGood observational and writing skillsMust have excellent listening skills with critical thinkingComputer skills requiredJob Types: Part-time, Per DiemPay: $45.00 - $50.00 per hourExpected hours: 3 - 5 hours per week to start, will increase as clients onboardBenefitsFlexible schedulePaid orientationProfessional development opportunitiesWork Location: In person (Portland metro)Equal Employment Opportunity StatementSeaStar Senior Services is an Equal Opportunity Employer.
